Faculty & Staff: Web Forms
Important
There are several methods of collecting data and using forms on the Scott College of Business webs pages. None of these methods should be considered secure and are subject to Indiana State University policies regarding the Family Educational Rights and Privacy Act (FERPA), Health Insurance Portability and Accountability Act (HIPAA), Institutional Review Board (IRB), as well as the university and OIT data collection policies.

Should regulated data be required or you are unsure of what can be collected please contact the Office of the Registrar or the Office of Information Technology
Webforms
Webforms are made in the website editor and are best suited for simpler forms. An email will be sent whenever someone fills in the form and the data collected can be downloaded either as an Excel or in CSV format. An example of a webform can be seen on the right. These type of forms are part of the web page.
Qualtrics
For more choice in the layout and design, types of content and logic within the form then a Qualtrics form is good. Qualtrics has several templates that can be used as a basis for your form or a new one can be made from scratch. The form can be made in either the college account or an individual account can be made by applying to OIT. As with the web forms, the data collected can be downloaded in various formats. The form is held on Qualtrics' servers and accessed is gained through a simple link on our web pages.
Printable Forms
Forms can be created in Microsoft Word and saved to PDF format. The PDF is then uploaded to our servers and a simple link on our web pages takes users to the document,
